These Forms may download and taken into your first counseling sessions:
1. Social History - for your child
2. Release of Information - if you believe that your counselor may need to talk with other professionals such as school personnel or a doctor.
3. HIPPA Privacy Rights - each client must have a form on file that states they received their privacy rights.
4. Accounting Registration Form - gives us the information we need to bill your insurance company and also allows the insurance to pay us directly.
5. Treatment Consent form - must be signed in order for us to provide treatment.
6. Klein Disclosure
